Bereavement leave
Employees may be absent without deduction from salary due to the death of a parent, grandparent, guardian, spouse, brother, sister, child, in-law, grandchild, step-child, step-parent, and step-siblings, or other member of the family who resides in the house hold (to include domestic partners) not to exceed five (5) days within a ten (10) calendar day period commencing on the date immediately following the death) for each death.
In addition to completing the form below (you will receive a confirmation of your submission), you must enter your absences into Absence Management. Your leave balance will be adjusted once documentation is received by HR.
Please be aware this process takes a minimum of one to two payroll cycles to be reflected on your paystub.
